By purchasing or authorizing the purchase of a ticket for a minor to attend the Beauty From Ashes Conference, I confirm that I am the parent or legal guardian of the minor listed in the registration. I understand that this event is organized by Beauty From Ashes and Breathe Life Ministries and will take place in Tampa, Florida.

I give permission for my daughter or the minor under my guardianship to attend the conference and participate in all scheduled sessions, discussions, and activities that are part of the teen program and general conference experience.

I understand that the Beauty From Ashes Conference is a faith-centered event where speakers may discuss topics related to faith, Christian beliefs, personal testimony, encouragement, spiritual development, and related themes. By completing this registration, I acknowledge that these topics are consistent with the mission and purpose of Beauty From Ashes and Breathe Life Ministries and I consent to my child being present for these discussions.

I acknowledge that the purchase of a ticket secures a seat for the minor to attend the conference sessions and that attendance at the scheduled sessions is expected as part of the teen program. I understand that it is my responsibility as the parent or legal guardian to ensure that my child attends the conference as registered and follows all event guidelines while present.

I understand that Beauty From Ashes and Breathe Life Ministries are hosting a conference environment and are not acting as custodial supervisors or childcare providers. I acknowledge that the parent or guardian retains full responsibility for the conduct, supervision, and well-being of the minor during the event.

I also grant permission for Beauty From Ashes and Breathe Life Ministries, along with their representatives, photographers, videographers, and media partners, to photograph, record, or capture video of conference activities in which my child may appear. I understand that these images or recordings may be used for promotional, educational, marketing, or archival purposes in print, digital media, social media, websites, or future conference materials without additional notice or compensation.

By completing this registration, I acknowledge that conference environments may include large gatherings of people, stage presentations, audience participation, and movement throughout the venue. I accept that participation in the conference is voluntary and that I assume responsibility for the minor’s participation in the event.

To the fullest extent permitted under Florida law, I agree to release and hold harmless Beauty From Ashes, Breathe Life Ministries, their organizers, volunteers, speakers, partners, venue hosts, and representatives from claims or liability arising from the minor’s participation in the conference except in cases of gross negligence or unlawful conduct.
